It is the current policy that all Lycoming College students participating in Intercollegiate Athletics provide the following information to help lessen your exposure to potential accident or injury hazards that may exist as a result of your participation in Intercollegiate Athletics.
The Lycoming College Athletics Physical has three parts.Â
1. The first part is the Athlete Information portal, which can be completed at:

2. The Doctor clearance form, which must be signed by a medical doctor, can be downloaded as a
.word doc here and a
.pdf document here.
3. Sickle cell documentation. There are no longer waivers available for sickle cell documentation. Student-athletes will need to provide documentation of their sickle cell status. This can be obtained through birth records with their pediatrician, the state they were born in, or blood work.Â
